In today’s healthcare marketplaces, stress
is experienced by most well meaning employees in health care settings. Nevertheless, when you or your family member are sick, or injured, you want to be appropriately taken care of, as is possible.

Serious questions, problems, or concerns can develop in an Emergency Room, Hospital, Critical Care Unit, Intensive Care Unit, Rehabilitation Unit, Nursing Home or Assisted Living Care environment.

Welcome Home Healthcare, Inc. can become involved.

To be of help, a Release of Information form must by obtained (by law).

This gives all parties concerned permission to communicate with one another.

This first step protects patient privacy, and protects the health care providers from breaking laws to protect patient confidentiality.

Discharge planning from an in-patient unit:

If the patient cannot go home, finding an acceptable, alternative environment for continuation of care can be very stressful or intimidating. With the patient’s permission (or his or her designee) Welcome Home Healthcare, Inc. provides intervention as the circumstances indicate.

Examples of Discharge and/or Health Care Planning Issues

Emphasize, through education, advocacy, and/or mediation, the patient-family perceptions and viewpoints of their identified problem(s).

Achieve patient-family goals to improve harmony and agreement and to decrease conflict and stress between all parties concerned.

Improve or resolve identified problems in a cost-effective fashion.

Increase the experience for all of being more in a “win-win” situation.

Other problem solving guidance and direction per patient/family identification or healthcare setting input.
